What is HDL particle testing?
Standard cholesterol tests measure how much cholesterol is carried in your HDL ("good cholesterol"). Advanced lipid testing goes further by measuring the number and size of HDL particles themselves.
Why particle details matter
Think of HDL particles as trucks that remove excess cholesterol from your arteries. What matters most isn't the size of the trucks—it's how many trucks you have and how well they're working. Research shows that having more HDL particles (especially small and medium-sized ones) is more strongly linked to heart protection than simply having high HDL cholesterol levels.
What about large HDL particles?
Interestingly, large HDL particles are not consistently associated with cardiovascular benefit. In some cases, very large, cholesterol-loaded particles may actually become less functional—like overfilled trucks that can no longer pick up cargo. This is why "more" or "bigger" isn't always better when it comes to HDL.
The bottom line
HDL particle testing provides a more detailed picture of your cardiovascular health than standard HDL cholesterol alone. However, the science is still evolving, and there are no specific treatments that target HDL particle size. The best ways to support healthy HDL function remain familiar: regular exercise, maintaining a healthy weight, not smoking, and eating a balanced diet.
