Leukocyte esterase reflects the presence of white blood cells in urine (pyuria).

Clinical interpretation:

• Suggests UTI or inflammation

• Sensitivity improves when combined with nitrites

Limitations:

• False positives: contamination

• False negatives: antibiotics, high glucose/protein, vitamin C

For healthy individuals: Isolated positivity without symptoms may not require treatment. Always interpret clinically.
